California was among one of the best states for senior healthcare in the United States, with high scores for prescription plans, coverage and more.
The Medicare Guide said “most people choose where to retire based on climate, proximity to family or cost of living,” but rarely look at healthcare access.

How good is senior healthcare in California?
California came in fourth place on Medicare Guide’s 2026 of the list of the best states for senior healthcare, with an overall score of 61.72 out of 100.
The guide said California’s Medicare Part D plan offers more options than any other U.S. state, giving “seniors exceptional flexibility in finding coverage that matches their prescriptions and budget.”
The Golden State ranked the highest in the nation for Medicare coverage and senior insurance plan benefits.
California’s Medicare Advantage plan has low out-of-pocket maximum costs on average compared to other states and Part D prescription drug plans have options to find tailored coverage for medications.
Medicare Advantage has three plans: Health Maintenance Organizations, Preferred Provider Organizations and Special Needs Plans.

HMO and PPO plans allow account holders to only use healthcare services within their respective networks, , according to California Health Advocates.
Some HMO plans have medication coverage while others do not. All PPO plans have prescription drug coverage.
Special Needs Plans are for people with certain chronic or disabling conditions, as well as those who are eligible for both Medicare and Medi-Cal and people who live in nursing homes or require the same level of care.
